ANQAS Application form is an application form applicable for applicant wish to enroll in ANQAS Certification Product Certification (PC) Scheme, where the applicant and/or product applied under applicant not yet certified by ANQAS Certification.
FI-03-01 3rd Party Application Form
FI-01 – 3rd Party Application form is an application form applicable for an applicant which to enroll in 3rd Party Inspection Service, where applicant wish to get attestation of Inspected Item is in accordance with applicant’s referenced document. Upon completion of 3rd Party Inspection, applicant will be supplied with 3rd Party Inspection report for applicant’s perusal.
FC-11 Amendment of Scope (KPS) Application Form
FC-11 – Amendment of Scope (KPS) Application form is an application form applicable for applicant, who already granted with Product Certificate, wish to extend and/or reduce (Amend) certification scope in the Product Certificate. Upon completion of KPS, applicant will be supplied with amended scope of Product Certificate.
FI-03-11 Amendment of 3rd Party Application
FI-03-11 – Amendment of 3rd Party Application form is an application form applicable for applicant, who already granted with Inspection or Verification Report, wish to amend the information. Upon completion of amendment, applicant will be supplied with amended Inspection or Verification Report.
WEPLS Testing Application Form
WEPLS Testing Application Form is the application form for service to conduct testing and provide laboratory test result for WELPS submission.
